How to Apply for Assisted Living
To make sure we can meet each applicant's unique needs, please complete the following four-part admission process.
Admission Process
Four steps to move in
Application & Financial Disclosure
Complete an application that includes a disclosure of income and assets. This is used to determine eligibility for our assisted housing program. You may also be asked to verify your income and assets.
Medical History
Your physician completes a medical history covering medications, health requirements, and personal care needs. This must be current (within 30 days of move-in) and received before move-in.
Interview & Assessment
A personal interview and assessment with our staff to evaluate the applicant's needs and confirm we can provide the appropriate level of support.
Background Check
A background check is completed on all applicants for housing at Your Home Wiggins ALF.
Financial Details
Deposits & Room Holds
Security Deposit
$1,000
Each resident is required to make a security deposit. This deposit is refundable when a move-in does not occur due to illness or failure to meet occupancy criteria, or at the time of move-out according to the terms of the Resident Occupancy Agreement.
Room Hold
75% of daily rate
To hold a unit prior to move-in, a room hold charge of 75% of the daily rate is required.
